Donation Submission Form

Thank you for all you do for Children’s Miracle Network and for your Children’s Miracle Network local hospital. The accurate submission of funds raised in support of the Children’s Miracle Network (CMN) is crucial for tracking our collective fundraising efforts. Please review the below details for submitting your CMN funds.

If you are in need of the CMN W9 for submitting donations, go to https://cmn.org/our-impact/financial-impact/. The form is listed under the More Information section at the bottom.

Event Checks

When submitting a check, please complete the donation form below indicating your resort name and how the funds were raised. Please include the the resort name in the memo of the check as well.

Sponsor Checks

If you are hosting an event and a sponsor would like to send funds directly to Children’s Miracle Network, please complete the form below to notify CMN that your resort or sponsor check is being mailed.

Please also ask your sponsor to include the completed form with their check to ensure the donation is properly credited to your resort and directed to your local member hospital.
